Highlights

  • The ideal pathway of care for those living with type 2 diabetes to prevent and manage CVD is a key feature of this consensus document. It offers a pathway of care that is the foundation from which to assess roadblocks that are context-specific.
  • A central feature is the importance of an integrated approach to care across settings, necessary to deliver high-quality care to those living with type 2 diabetes and CVD.
  • Collaborative action of all those touched by the growing epidemic of type 2 diabetes is needed to see a future downward trend of people living with this condition.
  • The roadblocks and potential solutions to patient care are offered based on key stages of the patient care pathway. They have been accumulated through an iterative process with the expert writing group and through a Delphi technique to a wider group of experts.
  • The patient perspective is the foundation from which this document is built. Patient stories and experiences can help to direct focus to areas of care.
